Home Health RN Hours

• 8am-5pm (M-F, no weekend and no on-call)
• Counties covered: Gaston County, Lincoln County, Cleveland County, and zip code, 28214

Home Health RN Compensation

• The pay for this position is $45.00-48.00 an hour paid weekly
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Home Health RN Responsibilities

• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Continuously evaluate appropriate, cost-effective levels of care
• Coordinate care transitions with physicians, social workers, discharge planners, and claims teams
• Arrange non-medical support services (e.g., housing, transportation) to support treatment adherence
• Engage specialty resources as needed to improve patient outcomes
• Maintain detailed documentation of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as a patient advocate while ensuring privacy and confidentiality
• Intervene promptly to prevent or minimize adverse patient outcomes

Home Health RN Requirements

• Licensed RN
• Home Health/Hospice experience or case management highly preferred
• Wound care/ wound vac, IV experience.
