Per Diem Physical Therapist – Mobile Division

Longevity Rehab Center

Longevity Rehab Center is seeking a motivated and reliable Per Diem Physical Therapist to join our Mobile Therapy Division. This role offers flexibility and independence, providing one-on-one outpatient physical therapy in patients’ homes and community settings.

Responsibilities:

• Deliver high-quality, evidence-based physical therapy to adult and senior patients in their homes.
• Evaluate, develop, and implement individualized treatment plans.
• Maintain accurate and timely documentation in accordance with Medicare and company standards.
• Communicate effectively with patients, caregivers, and the interdisciplinary team.

Requirements:

• Licensed Physical Therapist (PT) in the state of Florida.
• Reliable transportation and valid driver’s license.
• Strong interpersonal and time-management skills.
• Prior home health or outpatient experience preferred but not required.
