Job DescriptionPhysical Therapist – Home Health

We are seeking a dedicated and compassionate Physical Therapist with Home Health experience to join our growing team. This role is ideal for a clinician who thrives on providing individualized, patient-centered care in the comfort of patients’ homes.

Key Responsibilities

• Conduct comprehensive physical assessments and review medical histories to evaluate patients’ conditions accurately.
• Develop and tailor treatment plans that align with each patient’s rehabilitation goals and needs.
• Implement therapeutic exercises and training programs to enhance mobility, strength, and independence.
• Educate patients and families on injury prevention, recovery processes, and long-term wellness strategies.
• Monitor and document patient progress, adjusting treatment plans as needed to ensure optimal outcomes.
• Collaborate with physicians, nurses, and other healthcare professionals to provide comprehensive care.
• Maintain accurate and timely patient records in compliance with regulatory and organizational standards.

Qualifications

• Master’s degree in Physical Therapy.
• Minimum of 1 year of Physical Therapy experience in a Home Health setting (required).
• Current and valid state licensure to practice as a Physical Therapist.
• CPR certification (must be current upon employment).
