Care Management Coordinator at Providence summary: The Care Management Coordinator provides clerical and administrative support to the Care Management Department, coordinating communication among patients, providers, insurers, and other stakeholders to facilitate transitions of care, payment, and regulatory compliance. Requires at least 2 years administrative experience (healthcare preferred) and strong organizational, multitasking, and healthcare terminology skills. Description The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance. The goal of the Care Management Coordinator is to ensure the maintenance of accurate records, process documentation and support the delivery of quality patient care.
