Description:

Schedule: 40 hours, Monday – Friday 9:00am – 5:30 pm

Pay: $23.48 per hour DOE

Retention Payment: $3,000.00, with $1,000.00 payable at 12 months and $2,000.00 at 24 months

Position Summary:

The Housing Navigation Case Manager (HNCM) will work within a team to develop and implement housing plans for homeless guests, engaging with them to reduce barriers and develop individual housing plans that lead to safe, affordable housing. The role requires accurate data entry into Pine Street Inn’s Homeless Management Information System (HMIS), exceptional customer service, and partnership with other departments to provide integrated services.

The HNCM will operate based on principles such as:

• All people experiencing homelessness can achieve housing stability.
• Everyone is “housing ready,” regardless of sobriety, treatment compliance, or criminal history.
• Leverage guests’ strengths and connections to facilitate quick housing solutions.
• Recognize the impact of violence and victimization.
• Employ an empowerment model and maximize guest choices.
• Create a respectful, safe, and accepting environment.
• Focus on strengths, resilience, and cultural competence.
• Involve guests in designing and evaluating services.

Requirements:

Education/Training:

• BSW, BA, or BS in a human service-related field, or equivalent experience with a high school diploma plus three years in social services.

Preferred:

• Bilingual in Spanish/English and/or Haitian-Creole.
• Certification or training in counseling and/or case management.

Knowledge/Experience:

• At least two years of experience working with populations facing addiction and mental health issues.
• Strong computer skills, including MS Word and Excel.
• Experience creating housing and service plans for homeless individuals.
• Knowledge of housing opportunities and subsidy programs.
• Understanding of homelessness and housing issues.

Preferred:

• Experience with Motivational Interviewing, Trauma-Informed Care, Harm Reduction, Housing First, and Change Theory.
• Experience assisting with budgeting.
