Inpatient RN Case Manager at Providence Health and Services summary: Inpatient RN Case Manager provides care management for at-risk patients in an acute hospital setting, conducting assessments, care planning, discharge coordination, utilization review, documentation and patient advocacy to ensure appropriate resource use across the continuum. Required qualifications include an ADN/diploma, Alaska RN license, 2 years acute/inpatient experience, and competency in utilization review; Bachelors and specialty certification are preferred. Position is full-time, on-site at Providence Alaska Medical Center with standard nursing benefits, shift differentials, and relocation assistance for eligible hires. Description The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/ or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management RN is to ensure the use of appropriate healthcare resources throughout the continuum, so that the care provided is the right care, at the right time, in the right setting.
