We strongly live our value of a work-life balance. *Responsibilities* Comply with all state-mandated Occupational Therapy Practice Act, _Occupational Therapy Code of Ethics_ and other legal mandates. Evaluate client for therapy needs and provide written orders/treatment plan for services. Ensure plan of care is consistent with client assessment and needs. Instruct other personnel and/or family members and caregivers on intervention goals. Ensure proper visit documentation completion. Report clients response to treatment or changes in condition to DON, ADON and/or clients primary provider. Participate in discharge planning.. Provide in-service education for agency personnel, as needed. Ensure personal credentials are active and up to date at office. Attend all staff meetings and complete all required annual education as assigned. *Requirements* Current unrestricted Occupational Therapist license in state for which care is being provided One (1) year experience as occupational therapist, preferably in an in home setting CPR Certification, required Two-step negative TB skin test or chest x-ray, required.
