About This Role

We are seeking a highly skilled Patient Care Manager to lead our Home Therapies programs. As a key member of our team, you will be responsible for overseeing patient care and managing clinical operations to ensure the highest standard of care is provided.

Key Responsibilities:

• Manage multiple Home Therapies programs with direct responsibility for results, including growth, patient retention, operational efficiency, and cost management.
• Direct activities related to strategic plans, goals, and objectives, ensuring alignment with those of the region and division.
• Collaborate with operations leaders, market development, in-center leadership, and quality teams to promote home therapies growth in assigned programs.
• Ensure optimal performance of program operations to achieve or exceed key performance indicators.
