Job Description
1. Ensures quality and safe delivery of home health care services.
2. Oversees staff assignments and staffing patterns.
3. Provides guidance to assign appropriate staff to a client to meet the client’s needs.
4. Demonstrates sound judgment and clinical knowledge in planning and decision making.
5. Demonstrates competence in performing all nursing care, procedures and treatments.
6. Conducts initial client assessments and reassessments and develops plans of care.
7. Orients, supervises and directs field personnel in providing care.
8. Coordinates with community agencies to provide for continuity of care.
9. Responsible for discharge planning and the discharge process.
