Acting as a resource to patient/client, utilizes knowledge of community and health care resources to assist in addressing identified needs at any point of the continuum, pre-admission to post-discharge. Using timeliness and appropriateness of intervention, facilitates quality resource utilization. i.e., Length of stay and level of care.
