Job description
Case aide will assist case managers with case coordination. Case aide to assist with service agreements, moving coordination, and scheduling meetings. Case aide can assist with assisting person with completing paperwork, collecting documents from medical providers, attending medical appointments on behalf of the case manager with the person, and/or attend meetings with case manager. Case aide can assist with finding resources in the community, and placement for case manager. Case aide can assist with doing takebacks from providers.
Case aide will assist with all language translation and/or interpretation for all counties served in the agency.
Case Aide will assist with coordination with counties on forms, reassessments, and communication. Database Management: Create new files, placing paperwork in applicable folders, and maintenance of database information.
Case Aide will assist with all mail: Mail Distribution: Distribute mail items, scanning, emailing, and saving in database.
Case Aide will assist with auditing and tracking cases.
Case Aide will assist with all clerical tasks.
Case aide will assist with uploading and downloading required forms.
Case Aide to assist with all initial contacts for cases: all initial phone contact, mailing out paperwork, and coordination with county on new cases within the timeframe of the county they work with.
Serve as back up staff for other case aides, case managers and for empty caseloads.
Case aide to assist with coverage for other case aide when the other staff are not available.
Case aide to assist with phone coverage for front desk.
o Other Duties as assigned.
Required
o Applicants must have a valid driver’s license and acceptable driving record as determined by Bestview Care Options policy. Applicants must have a vehicle that is covered with current insurance. Applicant must have a reliable vehicle.
o Applicants must successfully clear local and/or stated background checks.
o Preferred Qualifications: One to two years’ experience in service coordination or case management, and clerical work is preferred, but not required.
o Must have a high school diploma or higher.
o Applicants will be required to have internet at home.
