Job Description

This Community Health Center is located in the RI/CT border. They follow a Patient Centered Medical Home (PCMH) model and believes in an environment that offers a work life balance. This Family Nurse Practitioner provides the full range of primary care. They will split their time between Hope Valley and Westerly offices.

The Nurse Practitioner responsibilities will include:

• Take Histories and Physicals Collect, record, and maintain patient information, such as medical history, reports, and examination results.
• Order, perform, and interpret tests and analyze records,
• Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
• Monitor patients’ conditions and progress and reevaluate treatments as necessary
• Counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter medications, and herbal remedies
• Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients’ individual circumstances

Qualifications:

• Graduate from an accredited FNP, DNP training program is required
• Active Rhode Island Physician, NP license in good standing
• Active DEA in good standing’ Current National Board Certification
• Minimum 3 years’ experience as family practice provider, emergency department or urgent care experience is preferred.
• Experience with women’s health issues, and medication assisted therapy is expected but not required at time of hire
