Under the direction of the Clinical Director, the Family Nurse Practitioner (FNP) will work to  provide direct nursing support to patients of Haak’u Health (HH). The FNP shall provide safe, direct patient care utilizing the nursing process in an ambulatory care setting. The FNP will provide clinical leadership for Primary Care staff and assist with other Primary Care duties as needed. The FNP will serve as a positive role model to promote a positive work environment and  provide quality patient care. Ensures adherence to department policies and procedures.  

ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES

This list of duties and responsibilities is illustrative only of the tasks performed by this position  and is not all inclusive.  

• Assesses the patient's condition and nursing needs, sets goals and utilizes appropriate  nursing actions to achieve goals  

• Provides direct health care services in an effective and professional manner. The services provided include but are not limited to: prevention/education, screening, evaluations,  counseling/therapeutic treatment, crisis response and other services which may be  required to offer comprehensive medical services 

• Ensures that direct patient services are provided in an effective, culturally appropriate  manner; Identifies and reports on problems/issues relative to access, barriers to services, and unmet needs;  

• Records and reports pertinent observations and reactions regarding patients to medical staff; Administers medications and performs routine and complex procedures and  treatments in accordance with privileging by clinical director;  

• Assists the attending physician with procedures and complex treatments;  • Assist with emergency procedures under the direction of the attending physician;  • Ensures that patient medical records contain the necessary information ( i.e., documents  

medical history of chief complaint, assists in triage and initiates patient care for walk ins.)  

• Works collaboratively with in-house and community providers for referrals and services;  • Documents labs, cultures, ensures follow-up on abnormal results;  

• Follows patient safety related policies and procedures;  

• Demonstrates a proactive approach to patient safety by seeking opportunities to improve  patient safety.  

• Identify and report conditions or situations which may put a patient at undue risk;  • Report potential or actual patient safety concerns, medical errors and/or near misses in a  timely manner; 

• Encourage patients to actively participate in their own care by asking questions and  reporting treatment or situations that he/she does not understand;  

• Partner with FNCH HIPAA Compliance Officer to ensure Primary Care is in full compliance with all HIPAA mandates;  

• Presents thorough information, data, and documentation required for administrative support and approvals;  

• Participates in community and inter-agency planning and implementation of programs, as assigned by the Clinical Director, to help improve coordination of services or  development of programs.  

• Other duties as assigned.

MINIMUM MANDATORY QUALIFICATIONS:

• Graduate from an accredited school of Nursing with the specialty as a Nurse Practitioner (Master's Degree Level).  

• Current, active & unrestricted license as a Certified Registered Nurse Practitioner.  • Two years experience as a Nurse Practitioner/Physician Assistant preferred. • Current BLS certification  

• Current ACLS and PALS certification.
